Our team of highly trained Licensed and Certified staff, coordinate to meet the demands of each client’s tailored care services plan.

SmartCare Software

SmartCare is a web-based electronic client and team member management program designed specifically for our Home Care business. With the ability to manage our clients’ Electronic Health Records (EHR), the program allows us to provide better care to our clients as well as make our jobs more efficient.

SmartCare Login

1) Navigate to the following site: https://UNIQUEID.smartcaresoftware.com/
    NOTE: The unique id for your branch will be provided in the welcome email for each user.

2) Drag the “Key” icon to the “Lock” icon. 

3) In the Credential Security box that appears, enter your User name and Password. Your personal login credentials would have been sent to you via company email at the time of your hire or when access was provided by a supervisor.

IMPORTANT: Your login credentials are unique to you and should never be shared with anyone else. Be sure to change your password if you feel it has been witnessed by others. If you feel your credentials have been compromised, please follow the appropriate Breach Notification protocol.

Because of SmartCare’s importance to our daily operations, we have dedicated a full page of resources created specifically for the employees that use it.

The section will expand and display the content available for that section. To watch a recorded session or open an interactive document, simply click on the link. For videos, a Media Player will open. For documents, Adobe will open.
